Please answer the questions in a sincere and thoughtful way, so that the project can count on the real opinion of the potential users. If the interested person is a child who is helped by the parents in understanding the questionnaire, I kindly ask the parents to support the answers of the child without trying to inspire an answer, because it could affect the final result of the survey. If the user is wearing a lower limb prosthesis it can be pointed out in the final section of the comments, so the answers/hints can be taken in consideration as well. The multi-answer questions are recognizable by a square check box. Those with a single answer are identified by a round circle. It is important to fill in the questionnaire only once. Any hint, observation, criticism or suggestion can be added in the appropriate field at the end of the questionnaire.
