How to get good answers

Send it in the last week, not at the exit interview. People say different things to a form than across a desk.
Send it from HR, never from the manager they are leaving.
Read the 'did you tell anyone' answer first. Somebody putting up with something in silence for a year is a process problem, not a people problem.
Collect them for six months before drawing conclusions. One leaver is a story, ten are a pattern.
