How to get good answers

  • Send it within two days, while people still remember the order things happened in.
  • Keep it anonymous. The useful answer is the one that implicates a decision, not a person.
  • Ask everyone involved, including people outside the team that caused it.
  • Your logs show what broke and when. They cannot show that somebody flagged it three weeks earlier and was not listened to.
