Evaluation of children hospitalization conditions from nurses' point of view

Dear fellow worker, Let me ask you for filling in this questionnaire monitoring conditions of children's hospitalizations from nurses' point of view. I am trying to identify issues accompanying children and their relatives during their stay in hospital, while I am focusing mainly on the category up to 15 years of age. The questionnaire is aimed for nurses working at in-patient departments providing care to children (including departments primarily determined for adult patients that receive also children patients for hospitalization in case of need). Filling the questionnaire will take you approximately 15 minutes, the questionnaire is anonymous, and by filling it in you agree with your participation on the research. There are no right or wrong answers in the questionnaire, therefore I please you to fill it in responsibly, if not stated otherwise, mark an option that complies best with your opinion. In advance, thank you for the time you will devote to filling in the questionnaire.
