Memories Questionnaire

Thank you for your interest in this survey! This study is part of a university artist's book project exploring how people recall and describe different types of memories. An artist’s book is a creative work that uses the form of a book to explore artistic expression.

Your responses will help provide insight into how personal experiences are expressed and remembered, and may be quoted and/or paraphrased within the contents of the book.

Your participation is completely voluntary, and all responses will remain anonymous. The information you provide will only be used for this academic project and personal portfolio work, and will not be shared or used for any other purpose.

By completing the survey, you are giving consent for your responses to be included in the research and project. However, you are free to skip any questions you do not wish to answer. Please write N/A in the question box if you do not feel comfortable answering.

If you have any questions about the survey, feel free to reach out to surveyqueries.memories@gmail.com. Thank you for sharing your experiences!
