What makes a successful intubated and awake assessment?

You see a patient for I & A. They are able to write on the whiteboard for key words. Handwriting can be variable but they are able to use partner assisted scanning and an alphabet chart consistently if you are unable to understand the writing. They can also access pictorial AAC well. Would you deem this a successful or unsuccessful I&A assessment?

You see a patient for I & A. They respond to all yes/no questions with a head nod and do not follow commands. They make no attempts to mouth and cannot engage in using any form of AAC. The recommendations you provide say "Please be aware that yes/no response is unreliable, with a preference for head nodding. Please anticipate all wants and needs". Would you deem this a successful or unsuccessful I&A assessment?
