Dear Sir or Madam, please take a few minutes of your time to complete the following questionnaire.

Recruitment Questionnaire for Osteopathic Study on Patient Expectations and High-Velocity Thrust (HVT)

Study title: "Assessing the impact of patient expectations on short-term outcomes of pain and functionality using high-velocity thrusts (HVTs) in osteopathic treatment."

Researcher: Benjamin Sibbons (University College of Osteopathy)

Thank you for your consideration in participating in this study, please read the following information carefully before you proceed.

Purpose of study: this study is aiming to explore what effect patient preferences in the type of treatment they receive has on the outcomes of that treatment.

What are HVTs: HVTs are a quick, but gentle, small movement applied to joints which is commonly used by osteopaths and chiropractors. A 'click' or 'pop' often comes from this movement which is completely safe and normal. HVTs are used for short-term pain relief and increased joint movement.

Confidentiality: Your information will remain confidential, anonymised, and securely stored. Participation is voluntary, and you may withdraw at any time without consequence.

Eligibility:

 - Age between 18 and 55 years

 - Currently experiencing musculoskeletal pain appropriate for osteopathic treatment

 - No known reasons against receiving/contraindications for HVTs (see below for details)

 - Able to consent to treatment and participate in the study

Contraindications for HVT treatment:

 - Recent fractures or severe trauma

 - Osteoporosis, arthritis or any inflammatory joint diseases

 - Active infections (inc. Tuberculosis etc.)

 - Significant neurological deficit (inc. Cauda equina, severe disc herniation etc.)

 - Blood clotting and vascular disorders (inc. aneurysm, vertebral artery insufficiency etc.)

 - Spinal cancers

 - Hypermobility or connective tissue disorders

There may be more contraindications to receiving HVTs, these can/will be discussed prior to treatment

Please acknowledge your understanding:

☐ I understand that the treatment I receive may or may not align with my preference due to the randomised study design.

☐ I understand I can withdraw my consent at any point without explanation or penalty.

☐ I am aware I will be asked to repeat the PSFS and pain assessment (VAS) immediately after treatment and again one month later.
